SMC d/b/a Shareable Forms ("us", "we", or "our") operates the Shareable Forms mobile application (the "Service").

Your username, First, Middle, and Last names are entered into the system both to uniquely identify you, and also to facilitate entering your identification information onto clinical forms you might fill out as part of using the system. Additionally you have the option of entering a mobile phone number to help when your password is forgotten or lost, however it is not required to do so.

By the very nature of the application as a system to record clinical data, we keep detailed history
about what actions you have performed using the system, including what kinds of activities you
have performed, which patient data you have recorded, etc.

If we become aware that we have collected Personal Data from children without verification of parental consent, we take steps to remove that information from our servers.
